Etiology and pathological mechanisms of recurrent bronchial asthma and efficacy mechanism of Jiedu Formula: A case report
支气管哮喘反复发作的病因、病理机制及解毒方疗效机制探讨——附1例报告

摘要

目的 探讨支气管哮喘反复发作的病因与病理机制,并阐释解毒方的作用靶点。方法 回顾性分析北京中医药大学东直门医院1例绝经后哮喘反复发作患者的临床资料,检测血清内毒素水平,预解毒方(绿豆、生甘草、黄连、苏叶、生姜、赤芍、白茅根、熟大黄、猪苓)治疗,随访3年余,结合文献分析内毒素与哮喘复发的关联。结果 患者女性,56岁。过敏性鼻炎7年余,绝经5年,绝经后首个冬季因煤气味刺激诱发喘憋,常规治疗仅短暂缓解,此后每年冬季复发,伴随失眠等症状,实验室检查提示血清嗜酸性粒细胞升高,治疗前血清内毒素55.57pg/ml(正常<5pg/ml),服用解毒方8剂后,喘憋、失眠症状消失,仅偶有打喷嚏,复查内毒素降至正常范围,随访3年余未复发。结论 血清内毒素异常升高可通过激活TLR4/NF-kB通路等诱发持续气道炎症,是哮喘反复发作的重要病因,解毒方通过多途径降低内毒素水平,为哮喘根治提供了新思路。

Abstract

Objective To explore the etiologies and pathological mechanisms of recurrent bronchial asthma, and elucidate the action targets of Jiedu Formula(Detoxifying Formula).
Methods A retrospective analysis was performed on clinical data of one postmenopausal female patient with recurrent bronchial asthma treated at Dongzhimen Hospital, Beijing University of Chinese Medicine. Serum endotoxin level was measured. The patient received treatment with Jiedu Formula (composed of Vigna radiata, raw Glycyrrhiza uralensis, Coptis chinensis, Perilla frutescens, fresh Zingiber officinale, Paeonia radix rubra, Imperata cylindrica, processed Rheum officinale, and Polyporus umbellatus). The patient was followed up for more than 3 years. Combined with published literature, the correlation between endotoxin and asthma recurrence was analyzed.
Results The patient was a 56-year-old female with a 7-year history of allergic rhinitis and 5 years post-menopause. Dyspnea was initially induced by coal gas irritation in the first winter after menopause; conventional therapy only achieved transient relief, and asthma relapsed every subsequent winter. accompanied by insomnia and other symptoms. Laboratory tests showed elevated serum eosinophils, Pre-treatment serum endotoxin was 55.57pg/ml (normal reference:<5pg/ml). After 8 doses of Jiedu Formula, dyspnea and insomnia were completely relieved, only occasional sneezing remained. Re-examination showed serum endotoxin decreased to the normal range, and no asthma recurrence was observed during over 3 years of follow-up.
Conclusion   Abnormally elevated serum endotoxin triggers persistent airway inflammation via activating the TLR4/NF-κB signaling pathway, serving as an important cause of recurrent asthma. Jiedu Formula reduces endotoxin levels through multiple pathways, which provides a novel strategy for radical treatment of asthma.
