Effects of exercise training on muscle injury in sepsis model rats
运动训练对脓毒症模型大鼠肌肉损伤的影响

摘要
目的 本研究旨在探讨运动训练对脂多糖(LPS)诱导的脓毒症模型大鼠血浆乳酸(Lac)水平、炎性因子(TNF-α、IL-1β、IL-6)、肌肉组织形态学变化以及肌球蛋白(myosin)和肌动蛋白(actin)表达的影响。方法 将50只雄性Wistar大鼠随机分为5组:对照组、模型组(LPS组)、模型+制动组、模型+制动+晚期运动组(第7天开始运动)和模型+制动+早期运动组(第3天开始运动)。对照组和模型组不进行干预,其他组通过石膏固定左前肢制动,运动组进行跑台训练(每天20分钟,持续至第14天)。实验结束后,采用ELISA法检测血浆指标,HE染色观察肌肉组织形态,Western blot检测蛋白表达。结果 与模型组相比,模型+制动组血浆Lac、TNF-α、IL-1β和IL-6水平显著升高(P<0.05),肌肉组织出现炎性细胞浸润和结缔组织增生,actin和myosin表达下降。早期运动组能显著降低炎性因子水平(P<0.05),改善肌肉组织结构,并提高actin和myosin表达,效果优于晚期运动组。结论 运动训练,特别是早期介入,可缓解脓毒症模型大鼠的肌肉损伤,其机制可能与降低炎症反应和促进肌肉蛋白表达有关。

Abstract
Objective This study aimed to investigate the effects of exercise training on plasma lactate (Lac) levels, inflammatory factors (TNF-α, IL-1β, IL-6), histomorphological changes in muscle tissue, and the expression of myosin and actin in lipopolysaccharide (LPS)-induced sepsis model rats. Methods Fifty male Wistar rats were randomly divided into five groups: blank, model (LPS), model + immobilization, model + immobilization + late exercise (initiated on day 7), and model + immobilization + early exercise (initiated on day 3). The blank and model groups received no additional intervention. The other groups underwent left forelimb immobilization via plaster fixation. Exercise groups performed treadmill running training (20 minutes daily until day 14). Plasma indicators were measured by ELISA, muscle histology was observed via HE staining, and protein expression was detected by Western blot. Results Compared with the model group, the model + immobilization group showed significantly increased plasma levels of Lac, TNF-α, IL-1β, and IL-6 (P < 0.05), along with inflammatory cell infiltration, connective tissue hyperplasia, and decreased expression of actin and myosin. The early exercise group significantly reduced inflammatory factor levels (P < 0.05), improved muscle tissue structure, and enhanced actin and myosin expression, with superior effects compared to the late exercise group. Conclusion Exercise training, particularly early intervention, can alleviate muscle injury in septic model rats, potentially through reducing inflammatory responses and promoting the expression of muscle proteins.
